This dataset originates from an institutional-based cross-sectional survey conducted between May 23 and June 30, 2018, in the Eastern Zone of Tigray, Northern Ethiopia. The study assessed the knowledge, attitude, and utilization of Adolescent and Youth-Friendly Health Services (AYFHS) among very young adolescents aged 10–14 years. Using a structured and adapted survey tool, data were collected from 628 participants (both male and female) selected through simple random sampling (SRS). The findings revealed low levels of knowledge and utilization of AYFHS in the region. Data analysis was performed using SPSS. The study was coordinated by the Tigray Health Research Institute under the Reproductive, Maternal, Neonatal, and Child Health program area.
